SY2022-23 Sports Physicals



TSSAA requires that all students participating in athletics have a sports physical form and a consent form on file with the school.

The forms must be completed before the student participates in summer practices, workouts, or contests. Forms must be dated after April 15; forms dated earlier than April 15 will not be accepted.

Our sports medicine partner, OrthoSouth, is offering sports physicals for $20 on May 18-19. You may also utilize your family doctor or nurse practitioner.
